• Enbridge Line 5Investigation ongoing

  645-mile, 30-inch crude and NGL line from Superior, Wisconsin to Sarnia, Ontario, in service since 1953. An August 25, 2026 NGL release of about 33,000 barrels at a valve excavation near Saxon, Wisconsin shut the line; it returned to service in mid-September on a temporary bypass the Wisconsin DNR says lacked state approval. PHMSA is investigating.

• Mangaluru Refinery (MRPL)Investigation ongoing

  MRPL's 300,000 b/d (15 million tonne a year) refinery near Mangaluru, India. A high-pressure cold separator in the coker hydrotreater ruptured on September 30, 2026, killing one worker; cause not yet established. A second injured contract worker died on October 17, 2026.

• Cardón RefineryUnconfirmed

  PDVSA's 310,000 b/d refinery, part of Venezuela's 955,000 b/d Paraguaná Refining Center, running far below capacity in 2026. Shut on October 6, 2026 after a gas line fire; accounts differ on which line failed.

• East-West Crude PipelineUnconfirmed

  Roughly 1,200 km crude line from the Abqaiq area in the Eastern Province to Yanbu on the Red Sea, with capacity near 7 million b/d, of which about 5 million is typically for export. Saudi Arabia's only large-volume export route bypassing the Strait of Hormuz; shut in mid-September 2026 after drone attacks on pump stations, restarted at a low rate on September 22 per Reuters sources, and back to about 3.5 million b/d by September 28 per Bloomberg sources. Attribution unconfirmed.

• Permian Gathering NetworkUnconfirmed

  West Texas gathering corridor under recurring satellite methane detections. Attribution disputed; no ground confirmation yet.

• Yanbu Crude Export TerminalsUnconfirmed

  Saudi Aramco's Red Sea crude loading terminals (Yanbu North and South), the western end of the East-West pipeline and the kingdom's main route around the Strait of Hormuz in 2026.
